BOOKING TERMS & CLIENT AGREEMENT

​

Hair Restoration & Replacement with Miss Kisha
A DBA of Braiding for Kids® LLC
(“Provider”)

This Agreement is entered into between the undersigned client (“Client”) and Hair Restoration & Replacement with Miss Kisha (“Provider”). By booking, scheduling, paying for, or receiving any service, the Client acknowledges that they have read, understood, and agreed to all terms outlined herein.

1. Consent to Services

​

The Client voluntarily requests and consents to receive cosmetic and beauty services, which may include but are not limited to:

• Hair restoration services
• Cranial prosthetics
• Hair replacement systems
• Hair extensions and sew-ins
• Wig construction
• Chemical services
• Braiding and styling
• Makeup services

All services are cosmetic in nature and do not constitute medical treatment.

2. Assumption of Risk and Release of Liability

​

The Client acknowledges that services involve inherent risks, including but not limited to:

• Allergic reactions
• Skin or scalp irritation
• Chemical burns
• Hair breakage
• Sensitivity to adhesives or products
• Infection or unforeseen complications

The Client voluntarily assumes all risks associated with services rendered.

The Client releases and holds harmless Hair Restoration & Replacement with Miss Kisha and Braiding for Kids® LLC, including owners, employees, contractors, and affiliates, from any and all claims, liabilities, damages, or losses arising out of or related to services provided.

3. Medical Disclosure

​

The Client is solely responsible for disclosing all allergies, sensitivities, scalp conditions, medications, diagnoses, or prior treatments that may affect services.

Failure to disclose relevant information releases the Provider from liability.

The Provider is not responsible for adverse reactions resulting from undisclosed conditions.

No refunds, exchanges, or redo services will be issued for allergic reactions or sensitivities.

4. Booking and Consultation Requirements

​

All new hair restoration or alopecia clients must complete required intake documentation prior to scheduling.

Consultations are scheduled only after review of intake forms.

Virtual consultations require the Client to be on camera in adequate lighting with natural hair visible.

Arrival more than fifteen (15) minutes late may result in cancellation without refund.

5. Deposits, Payments, and No-Show Policy

​

A non-refundable deposit is required to secure any appointment or consultation.

• Deposits may be transferred once within thirty (30) days, subject to availability.
• After thirty (30) days, deposits are forfeited.
• Cancellations must be made at least forty-eight (48) hours in advance.
• Failure to provide proper notice results in forfeiture of the deposit.
• Failure to appear may result in an additional charge equal to the full service amount to the card on file.

Certain specialty services may require full payment in advance.

All sales and services are final.

6. Cancellations and Rescheduling

​

A minimum of forty-eight (48) hours’ notice is required for cancellations or rescheduling.

Late cancellations or tardiness exceeding fifteen (15) minutes may result in forfeiture of deposit and possible full-service charge.

7. Custom Orders and Specialty Services

​

All cranial prosthetics, custom hair systems, specialty extensions, and custom-ordered materials are final sale.

Once ordering or production has commenced, cancellations and refunds are not permitted.

​

8. Client Responsibility

​

The Provider is not responsible for:

• Ill-fitting wigs due to inaccurate measurements provided by the Client
• Incorrect extension length, color, or texture selected by the Client
• Manufacturer defects
• Damage caused by improper aftercare

 

Maintenance schedules must be followed. Results vary based on scalp condition, lifestyle, and compliance.

 

9. Media Release

The Client acknowledges that photographs, video recordings, or digital media may be captured before, during, or after services.

​

Such media may be used for marketing, promotional, or educational purposes unless the Client declines consent in writing prior to service.

​

No compensation shall be provided for media use.

​

10. Guest and Minor Policy

​

No additional guests or children are permitted unless receiving services.

The Provider reserves the right to refuse service to maintain safety and professionalism.

​

11. Limitation of Liability

​

To the fullest extent permitted by law, liability of the Provider shall be limited to the amount paid for the specific service in question.

​

The Provider shall not be liable for indirect, incidental, special, consequential, or punitive damages.

 

12. Payment Disputes and Chargebacks

​

By submitting payment, the Client acknowledges that submitting payment constitutes acceptance of this Agreement.

The Client agrees not to initiate a chargeback, payment dispute, or reversal for services rendered, deposits paid, custom orders initiated, or specialty products purchased in accordance with this Agreement.

In the event of a chargeback or dispute:

• This Agreement and all booking documentation may be submitted as evidence.
• The Provider reserves the right to pursue recovery of disputed funds, including administrative fees, arbitration costs, and reasonable legal fees.
• The Client may be permanently restricted from future services.

13. Dispute Resolution and Arbitration

​

Any dispute, claim, or controversy arising out of or relating to this Agreement or services provided shall be resolved exclusively through binding arbitration in the Commonwealth of Virginia.

​

Arbitration shall be conducted under the rules of the American Arbitration Association or comparable provider selected by the Provider.

The Client:

• Waives the right to trial by jury
• Waives participation in class or collective actions

Judgment on the arbitration award may be entered in any court of competent jurisdiction.

​

14. Governing Law

​

This Agreement shall be governed by the laws of the Commonwealth of Virginia, without regard to conflict of law principles.

​

15. Binding Effect

​

This Agreement is binding upon the Client and their heirs, representatives, and assigns.

By booking services, the Client confirms they have read, understood, and agreed to all terms herein.
